Describe the issue precisely
Start with evidence rather than a general ambition. “Improve learner support” is too broad to evaluate. A more useful statement explains which learners experienced what difficulty, over which period and how the institution knows.
Include limitations in the evidence. A low-response survey may indicate an issue worth investigating, but it does not establish what every learner experienced. Combine appropriate sources and avoid reporting a small sample as an institution-wide certainty.
Choose an action and measure
Give each action a named owner, a deadline and a measure connected to the problem. Distinguish implementation measures, such as publishing a support procedure, from outcome measures, such as whether learners receive responses within that procedure’s stated time.
Make the target proportionate to the institution’s capacity and the needs of its learners. If several teams are involved, identify who has final responsibility and how progress will be reported.
Worked example: support response times
Illustrative example, not a member case study. An internal review finds that 18 of 60 support enquiries exceeded the institution’s stated response time. The institution first checks whether the records cover comparable enquiries and whether the measurement is reliable.
The support lead introduces a shared enquiry log and assigns responsibility for unanswered requests. The plan records an implementation date and a review after eight weeks. The outcome measure is the proportion answered within the published time, supported by the log and a small check of learner feedback.
The evaluation should report the actual result and any remaining limitations. Creating the log completes an action; checking whether response times improved completes the improvement cycle.
Keep completed work visible
Record the completion evidence, evaluation date, result and next decision. If the result did not improve, examine why and revise the action. Avoid deleting unsuccessful actions from the record.
For membership, a short annual update on one priority is enough. Accreditation uses a one-page plan with two actions and feedback under A06. More detailed tracking is optional.
